The impact of providing enhanced support for care home residents

At the recent NCF event on Unlocking the Potential of Data in social care, we highlighted how effective data analysis could effectively argue for the impact of changes in service delivery and designed. 

This research carried out by the Health Foundation shows how a model of enhanced support offered over a group of care homes could be analysed and benchmarked across a group of comparative sites, and then to show how key metrics drawn from shared data between hospitals and care homes, showed significant reductions in accident and emergency and hospital admissions. Alongside the importance of the data, there is also a very clear exploration of what the enhanced support consists of, and a shopping list of changes to be made to facilitate these improvements including greater support for care home nurses from community nursing staff, access to independent advocacy and a programme of support for care home managers.

This makes for a very interesting report which members may wish to use to demonstrate integration potential within localities.
