Community Manager role at St Luke's Hospice

At St Luke’s Hospice we are committed to delivering excellence in care. Our service users are at the centre of everything we do. We work in partnership with service users and colleagues from other organisations to achieve the best outcomes we can. 

Our community services have gone through a period of sustained growth over the last few years. Our caseload has increased and the range of services we deliver within the patient’s own home has broadened. We see partnership working as a key component of our success and the integration of services, vital to the delivery of patient centred care.

We are now seeking to appoint a Full Time Community Service Manager. This is a new senior clinical role within the hospice. The role will provide coordinating leadership for our own community services, directly managing the 4 Lead Nurse Nurses within the Community Service; and support our ambition to work in an integrated way with partners to deliver patient centred care. Our current community services include the Community Palliative Care Team for North Brent, our Brent Care Home Service, the Hospice at Home Service for Brent and Harrow and the Harrow Single Point of Access and Rapid Response team. You will lead our Community Services into the next phase of our development, working with partner organisations to deliver end of life care to our communities.

You are a Registered General Nurse with significant experience of community working. Ideally this will have been within the field of palliative care or a related clinical discipline. 

As a senior clinician you will promote and influence excellent end of life care within the hospice and the locality we serve. You will be committed to multi-professional working and the delivery of high quality care to our patients and families. Attendance at relevant external meetings and events will be required.
